What should I bring to my initial appointment?
At the time of your first consultation you should bring:
A referral from your general practitioner or referring doctor. This enables you to be seen by the appropriate specialist to treat your health needs as well as receiving a higher rebate from Medicare.
Medicare and private health fund information.
A list of all medications you are currently taking.
A list of any known drug allergies.
Relevant x-rays, reports and test results. On arrival please give all your results and letters to our staff for scanning.
Please ask your GP to fax the referral to our rooms (or send it yourself) to allow us to prioritize your appointment appropriately.
If you have any queries or concerns, please do not hesitate to call our staff who will be happy to answer any questions you may have.
Patient Privacy Policy
North Sydney Specialists respects and upholds the rights of all our patients to privacy protection under the National Privacy Principles contained in the Privacy Act 1988. The National Privacy Principles apply to us from their introduction on 21 December 2001. The Privacy Principles set the standards by which personal information is collected, handled, used and disclosed. As part of providing a quality health care service, we need to take and maintain information of a personal nature in your medical file and we will not release the contents of your medical file without your consent.
